Your pet's medical records
See your pet's records online in My Pet Avenue, or ask us for a copy of the full records.

See records online
My Pet Avenue is our client portal. Log in with the phone number or the email address that the clinic has for you. You can see:
- Your pets and their details.
- Vaccines and preventive care, with the due dates.
- Weight history and prescriptions.
- Upcoming and past appointments.
- Invoices.
No account yet? Book online or ask us. If you are a client and the login does not find you, send us a WhatsApp message and we update your details.
Ask for a copy of the full records
Send a WhatsApp message or an email to petavenueclinic@gmail.com. Send it from the phone number or the email address that we have on file. Tell us:
- Your name and your pet's name.
- The records that you need (for example, all records or the vaccines only).
- Where to send them: to you, or to another vet (give the vet's email).
We check that you are the owner before we send records. Then we send them by email.
Records from another vet
New to Pet Avenue? Bring your pet's old records and vaccine book to the first visit, or ask your old vet to send them to petavenueclinic@gmail.com.
